The Liability/Medical Release forms are required for ALL participants attending the Steubenville East 2011 Conference:

Teen Liability/Medical Release Form must be completed and returned to Tom Wimer before participation in conference.

ADULT Liability/Medical Release Form for participants age 21 and older.

Expectation of Participants - Each participant is expected to adhere to these principles while attending Steubenville East Conference.
